Reporting Requirements

A report on any approved and funded grant application will be due at the end of the one year grant cycle (as set forth in the contract letter). 

The grant report should consist of two parts:

  1. A narrative of the program/project’s goals and actual accomplishments

  2. A separate income and expense statement showing how the grant funds were spent.

The report may be formatted as the grantee wishes. In the event a program/project is not completed within the one year grant cycle, an interim report is required. If the program/project is completed within the one year grant cycle, all grant funds have been spent and a full report can be made, the report may be submitted at any time.  Grant reports may be submitted by either email or regular mail.

Re-Grant Policy:

The Spaulding Foundation’s re-grant policy is three-fold:

  • The submission of an acceptable grant report as outlined above (if applicable)

  • One year between grants

  • Not more than three grants in any five year period

Application Process

Review our guidelines to determine if your organization and proposed project/program fit the focus and criteria of The Spaulding Foundation. Please contact us with any questions.

Request a grant application form. Pages may be added to the application form in order to include all relevant information. Please also note the additional documentation that must be submitted with the application. Failure to provide all requested information will result in an incomplete application and it will not be presented to the Board for consideration.

Submit the grant application and all required documentation per the grant calendar below. Email submission is preferred; applications sent by regular mail must be postmarked no later than the application deadline.  Do not use any delivery method requiring a signature.

The Grant Administrator may follow up the application with a request for a meeting or a site visit.

Applicants will be notified of the Board’s decision by mail within one (1) week of the Board meeting. If the grant is approved, a contract letter will be mailed for signature and return by the appropriate personnel.
